Overview

Our GP & Hospital Primary Care Liaison Unit provides assistance to Austin Health staff, the GP community and primary care agencies to develop links and opportunities for collaboration.

We are keen to provide contact details, referral information and forms to make it as easy as possible for GP and health professionals to refer their patients to Austin Health.

Rehabilitation referrals

Heidelberg Repatriation Hospital (including Geriatric Evaluation Management)

Heidelberg Repatriation Hospital specialises in Geriatric Evaluation Management (GEM), aged care, orthogeriatric rehabilitation and neurological rehabilitation.

Referrals to this service require completion by a geriatrician.

Royal Talbot Rehabilitation Centre (including Acquired Brain Injury)

The Royal Talbot Rehabilitation Centre (RTRC) specialises in general, amputee, ortho and neurological rehabilitation, and Acquired Brain Injury (ABI) and spinal rehabilitation.

Referrals to these rehabilitation services will be reviewed at Austin Health by the relevant medical consultant.

Please email completed referral form below to externalreferrals@austin.org.au and attach latest pathology, observations relevant imaging and medication chart.
