About your Health Questionnaire
When you are added to the preparation list (this is sometimes called a waiting list) for a planned procedure at Austin Health, we will ask you to complete your Health Questionnaire.
Your Health Questionnaire is a detailed form that collects information about your medical history. It covers:
- past procedures
- medications
- allergies
- and other relevant health details.
This makes sure your care team has all the information they need to safely plan your procedure and provide you with the best care.
We'll also ask you to update your form from time to time to help us detect any changes early and ensure they don't impact your care.
