About us

Our COACH (Coaching On Achieving Cardiovascular Health) program provides a telephone-based coaching service to support people with heart disease to manage their cardiac risk factors.

Our services

We provide the following services:

• assessment of cardiac risk factors
• education on lifestyle strategies to assist patients to achieve and maintain their cardiac risk factors targets
• encourage patients to consult with their doctor/s regarding medication and regular measurements of their risk factors.
