We support people with disability to use hospital services and access the care and treatment they need.

About us

Our services

We support people with disability, their families and support networks to access:

  • Austin Health services and other health and human services
  • inpatient and outpatient services
  • COVID-19 testing, treatment and vaccination

We also provide advice to health care teams to provide safer, accessible, and more inclusive care.

Autism Care Plan

The Autism Care Plan is a way for you to tell Austin Health about your needs, likes and dislikes. This care plan will help Austin Health provide you with individualised treatment and care when you stay at hospital.

You can fill out this care plan by yourself, with help from someone else or a Disability Liaison Officer before you come to Austin hospital.

Once you have completed the Care Plan, please send it to the Disability Liaison Officer team. We will save it in your medical record for when you come to Austin Health. You can also bring a copy of the Care Plan with you when you come to Austin Health.

Your rights at hospital

People with disability have a right to get safe healthcare like everyone else. Our hospital has rules to make sure you get the healthcare you need.

For more information about your rights as a patient, including translations in different languages, see: Your rights & responsibilities.

Patient care preferences

Patient care preferences tells us who and what is important to you, and ways in which you would like to receive your care.

When admitted to hospital, we will ask you some questions about yourself. These questions include what your day-to-day activities are, usual behaviours for you, how you communicate, how you move around and who your support people are.

By letting your healthcare team know this information, it means that we can provide care in a way that suits you.

If you are planning to come to hospital, you can answer these questions before arriving. You can do this by completing our patient care preferences form below.

Alternatively, we can ask you these questions when you arrive at hospital.

Please contact the DLO team if you need any further information or would like support to complete your care preferences.
