What we do
Our DCAS service is a multidisciplinary team of health professionals with expert knowledge and experience in diabetes management including endocrinologists, diabetes nurse educators, dietitians, and allied health.
We provide the following services:
- Supporting patients to assess their ability to self-manage their diabetes
- Diabetes education
- Diabetes complication prevention
DCAS is part of the suite of services provided by the Health Independence Program (HIP).
What to bring
When you come to hospital for your appointment, you need to bring:
- Medicare card
- Medication you are taking
- Your blood glucose monitor and lancet device
- Your blood glucose readings or log book
- Pathology results or name of the pathology company where you last had blood tests
- Glasses/hearing aid
- Support person
