When to use this form

Use this online form for:

• Appointment bookings – Have you received a letter asking you to book an appointment? You can request a booking via our online form and our staff will make an appointment for you. They will send the booking confirmation as an appointment letter in the mail.
• Changing appointments – You can request another date, but please note that most clinics are in very high demand and we are unable to issue a new appointment date immediately. You might return to the waitlist for a future appointment to be offered. These wait times will vary between clinics.
• Cancelling appointments – If you cancel, you will return to the waitlist and receive a future appointment when available. If you don’t want any further appointments, let us know and we’ll remove you from the waitlist.
• Waitlist enquiries – We manage referrals in turn via waitlists. We’ll send you an SMS when it’s your turn to be seen. If you have any questions about your waitlist, please submit an enquiry online.

You can only contact us for appointments in Specialist Clinics. Booking enquiries for other services should be made directly with the service.
