Contact our intake worker
Our highly trained clinicians can respond to a diverse range of queries concerning people with acquired brain injury or neurodegenerative disorders. Your first step is to contact the Brain Disorders Program (BDP) intake worker.
Our intake worker can be contacted from Monday to Friday between 12.30pm and 4.30pm.

How to make a referral
We accept referrals from a variety of sources including individuals, family members, carers, support coordinators and health professionals. You can submit referrals using the referral form below via mail, email or fax.
Referrers will need to provide supporting health information or details of where this information can be obtained. Documentation of the brain injury is required, for example:
- neuropsychological assessment
- rehabilitation discharge summary
- medical imaging such as MRI.
The individual may be advised to see their GP or other local services prior to referral, however our intake worker is available to discuss the situation and make a recommendation based on your individual circumstances.
